What causes the Output Fader to turn red and stop controlling the Main Outputs?​

Answer:

The Fader and the Monitor output controls have been swapped to use the mixer in a Monitor console configuration.  To change back to default:

• Select the Menu button
• ​Select Monitor Setup page
• Select LR/Monitor Swap and disable it

This should return the Output Fader to normal operation.
